  • Responsibilities

     The Case Manager directly coordinates and monitors a complete rehabilitation program to ensure the clients and families receive planned and efficient interdisciplinary treatment services. Functions as an effective team leader. Writes client progress reports, and is responsible for all communication regarding client and program information to insurance carriers, attorney(s), accounts, family members, physicians, and treatment team members. Responsible for evaluating clients and developing plans of care with measurable short-term objectives and long-term goals.

     

    Core Qualifications: 

    Bachelor's degree or higher in Psychology, Social Work, Nursing or Special Education

    Willingness to be on-call

    Leadership skills

    Strong written and verbal communication skills

    Strong interpersonal skills

    Ability and willingness to learn new skills

    Ability and willingness to set priorities

    Ability to work as part on an interdisciplinary team and serve as a team member

    Ability to lift, maneuver and carry up to 30lbs.

    Valid driver's license

    Acceptable driving record

    Acceptable under the State of Michigan Good Moral Character Statute

    Knowledgeable of HIPAA Privacy Rules

     

    Additional Qualifications:

    Previous Medical Case Management preferred

    Experience working the TBI population preferred

    Previous experience with Children and Adolescents
