The Home and Community Physical Therapist is responsible for evaluating clients, providing clinical treatment and implementing individual client programs in their homes and in the community, as well as formulating a comprehensive treatment plan with other interdisciplinary team members.
PT would work in the home health setting (no OASIS/Medicare... Payor is Auto) for the counties of Washtenaw, Oakland and Wayne County. Occasionally the PT may also cover in our Genesee Treatment Center. A schedule could consist of 50% in the community and 50% in the clinic. PT is compensated mileage* for anything outside their normal commute to work (*point would be determined at hire). Electronic documentation is place; prospective clinician would be issued a computer and phone.
Licensure to practice physical therapy in the State of Michigan
